1 School-Based Clinics and Family Resource Centers
The Organization
Facilitator (OF) serves as liaison between the Division of Health and
Human Services and schools for the purpose of assisting in the development
of coordinated health and mental health related services. In partnership
with District 1 Schools, community and families, the Organization Facilitator’s
primary goal is to reduce the barriers to student achievement and foster
increased student success.

The Organization
Facilitator provides the following resources and services to the
District 1 Schools:
Coordinates School-Based
Health Clinics and Family Resource Centers
- Coordinates
the delivery of site based health, mental health, and social services
by facilitating the MOU/Contract process
- Develops
school site delivery plans
- Collects
compliance items and submit applications
- Facilitates
projects that improve Health and Human Services for students and
families
- Assists
with Local District Crisis Team training and implementation
- Initiates
and coordinates grants/funding development
- Procures
health/mental health and social services resources
- Interfaces
with Health Education Programs, Impact, TUBE and Tobacco Prevention
Advisors and Coordinators
- Works
closely with Local District Nursing, School Mental Health, Special
Education, Psychological Services, PSA and Operations Coordinators.
Resource
Coordinating Council
- Council
meets bi-monthly on Thursday afternoons at different
District 1 school sites
- Council
meetings are open to all interested school and community agency
personnel
- Participation
from school and agency representatives working in the student health
and human
service fields is encouraged
- Facilitates
collaboration with city, county and community agencies on health
and mental health
issues
- Develops
needs assessments, strategic plans, community resource information
related to health and
human services
- Provides
professional development programs for health and human service
personnel and fosters
networking with health related city, county and community agencies
- Sponsors
annual District 1 Resource Coordinating Council Retreat

- Provides
Local District public relations
- Disseminates
health information
- Connects
academic supports
- Coordinates
revenue development for school based clinics & resource centers
- Develops
and assists with health education programs
- Connects
to Federal, State & County Agencies
- Participates
in collaborative health and mental health partnerships
