The Division of Special
Education has established the District Office
of Transition Services to work closely with middle
school and high school staff serving students
with disabilities, and to provide transition services
to all students of transition age.
Each comprehensive high school is assigned Transition
Teachers to provide and support a complete array
of transition services to students. The Transition
Resource Teacher (TRT) and the Transition Services
Teacher (TST) work in collaboration with school
staff to conduct professional development and
other activities required to provide compliant
transition services in accordance with IDEA.
The Transition Services Teachers support the coordination
of transition assessment, instruction, and activities
for students with disabilities. The TSTs are charged
with implementing legal mandates and District
guidelines for the provision of compliant transition
planning and service delivery. They provide training
for school staff (including those at middle schools)
on the development and implementation of compliant
Individualized Education Programs and Individual
Transition Plans, and assist classroom teachers
in providing families of students with transition
planning tools.
The Transition Resource Teachers assist students
with disabilities and their families with connections
to community agencies, departments, and work experiences.
The TRTs coordinate and conduct assessments to
determine eligibility and access to various community
agencies or services. They facilitate and/or monitor
community linkages for students, in addition to
advising and providing linkages to special education
students in work/career areas including special
schools, continuation schools, Centers for Advanced
Transition Skills, and Occupational Centers.
Finally at each of the four Support Units,
a two Transition Coordinators are responsible for providing
technical direction and support to Local District
staff, Transition Teachers, and school staff in
transition services for students with disabilities.
